Caris Cohen Dx is happy to send slides to outside institutions when the patient’s follow-up care mandates.

In an effort to streamline the process of releasing a patient’s medical materials for continuing care, we have modified our policies. We no longer require patients or their referring physician to sign the “Authorization for Release of Medical Information” form. Instead, we have devised a new “Request for Medical Materials” form. This form can be completed and signed by the patient/guardian, the patient’s referring physician or the physician requesting the material.

Previously we did not accept requests that were signed by parties other than the patient/guardian or the referring physician. By completing the form on behalf of the patient, the requesting physician (or the patient care coordinator) must attest that s/he has been authorized by the patient to receive the material. We will not accept a form that does not contain a signature and legibly printed name.

Our preferred method remains to secure the patient’s signature on our form, but we hope this new flexibility will facilitate the patient’s continuing medical care in circumstances where time is of the essence.

All packages will be sent via UPS Ground service, at our expense. UPS Ground service guarantees delivery within 1-5 business days. If you require expedited service, please contact us to make alternative arrangements.

Patients are also welcome to pick up their slides at our office and can complete a request form in-person when they arrive to pickup the slides. We request that all patients provide at least two hour’s notice when coming in to pick up slides.
