GETTING STARTED WITH ONLINE REPORTS
Setting up access to the Caris
Cohen Dx Online Reporting service requires participating clinicians to complete the “ORV Access Request and Authorization Form” and return it to Caris
Cohen Dx. Access is established within one week. Click here to get a downloadable copy of this form, print it out, and follow the instructions below:

Step One:
Complete the “ORV Access Request and Authorization Form”:

This form allows you to request a user name and password for the Caris
Cohen Dx Online Report Viewer. Every provider wishing to use the ORV will be set up with his/her own user name and password. For security and account management reasons, we do not provide generic user names to be shared among multiple providers. However, an individual provider can have access to the pathology reports for other members of the same practice. A single user name can be set up to include rights to reports for multiple providers.
Additionally, you can state your service preferences once online reporting is activated. You can elect to stop receiving reports via UPS, courier or fax and exclusively use the web to obtain results. Or, you can opt to continue receiving hardcopy reports and use the Online Report Viewer in conjunction with your current methods.
Most importantly, this form grants Caris
Cohen Dx permission to create and post electronic copies of patient reports to the web. In compliance with HIPAA, it instructs users of Caris
Cohen Dx ’s online reporting feature to carefully safeguard their username and password to ensure the security and privacy of patient data.
Step Two:
Send the forms back to Caris
Cohen Dx via fax (617-969-3393) or courier.
Step Three:
Caris
Cohen Dx will establish access and assign a username and password. This information, along with an instructional packet, will be sent back to your office within one week.

Upon receiving the Online Reporting packet, you may immediately start accessing the Caris
Cohen Dx web site for reports.
The availability of online reports begins from the date on which the username and password are assigned. The online report database does not contain a running history of all reports. Reports only date back to when you became an active user of the Online Reporting feature. From that point forward, reports will remain in the system indefinitely. Reports are posted to our secure web server every 30 minutes between 10:00 am and 7:00 pm. |
